1.4 - By assessing the situation, and implementing a plan initially we can quickly establish what needs to be done. By doing this we can see any barriers, which may arise. When dealing with a client a plan of care will identify the problems and what they would like to achieve. By working with everyone within the multidisciplinary team appropriate assessments with Speech Therapists for example can arrange the necessary aids, training if necessary for staff to understand food consistencies, how the client is to be seated and that they are mindful of the clients needs.

Working with all professionals, interpreters, audiologists, GP’s etc, as a manager ensuring staff are adequately trained in all areas and mandatory training is up to date can eradicate many issues.

3.3. Propose improvements to communication systems to support partnership working.
Collating or collecting information is essential in monitoring the effectiveness of the communication systems. We can collect the information observing, getting feedback from others and recording information. So we need to work with others to identify ways to support the continued development of communication.
Care professionals communicate with colleagues and other professionals in many contexts every day. Effective communication requires personal and professional respect for others, trust in the judgement and values of colleagues, good verbal and listening skills.

Being able to communicate effectively builds trust, respect, enhances learning and accomplishes goals. Within my job role the level of communication I have to uphold has to be one of massive importance.

There are a wide range of individuals that I communicate with on a daily basis i.e. clients, support workers, mangers, clients parents, gp’s. Which are different to one another for example if I’m communicating with a client I would use their preferred communication method wherever it would be Makaton or pictorials all depending on which is their preferred means of communication. If I was not to use their preferred means of communication this would cause a barrier and possible upset that that individual. When communicating with support workers to carry out a task I would need to talk clear and slowly so that person could understand instructions and always make sure they understand what the task is I am requesting them to do. I am happy to listen to any concerns that the may have or any issues in the work place or problems they may be having in their personal life. I am happy to use and visual or hearing aids if they require that support. When talking to a manager I will show them respect throughout the conversation and not undermine anyone. I use emails on a regular basis to get in formation to people that do not work at the home for example occupional therapists, Physios. But emails can be sometimes be miss read so I will always make sure they can understand it by a possible follow up phonecall.Communication can sometimes be miss read so I try and always make sure everyone can understand.…
