The compliance process is critical when it pertains to the billing and coding process. It is imperative for Medical Insurance Specialist to remain current on the patient’s participation in contract as well as the medical insurance policies, so there will not be any billing errors. Maintaining a communication with the payer will also prevent billing errors. Such regulations and laws are in place for to protect the patient’s financial state, prevent errors of billing and coding, and to link procedures and diagnoses correctly. Becoming knowledgeable of the billing rules should also help prevent billing errors as well. In this assignment, I will discuss how important it is for medical staff to be knowledgeable of the billing and coding compliance strategies.
The first strategy to compliance is to carefully define bundled codes and know global periods. This is important, medical insurance specialist must be knowledgeable of what global periods are for surgical procedures and what specific procedure is included in the bundled codes. The second strategy is to benchmark the practice’s E/M codes with national averages. This strategy helps when conducting comparisons with the national averages, because it helps to monitor upcoding. The third strategy is the use of appropriate modifiers. This strategy helps prevent double billing or unbundling. Modifiers such as -25, -59, and -91 are most important when billing in compliance. The fourth strategy is becoming clear on professional courtesy and discounts to uninsured/low income patients. Unfortunately, providers can no longer provide professional courtesy to patients per OIG’s Compliance Program Guidance for Individual and Small Group Physician Practices. However, if the patient may qualify for discounts if they are either uninsured or has low income.     In the article titled, “The risk and consequences of clinical miscoding due to inadequate medical documentation: a case study of the impact on health services funding” the authors proved through an audit of medical records, that the biggest financial losses were a direct result of either improper clinical documentation or lack of documentation altogether. The objectives in conducting both an internal and a blind audit at a hospital in Melbourne, Australia were to measure discrepancies in clinical coding, identify financial loss associated with diagnosis related group (DRG) changes, reveal errors associated with improper documentation, and to suggest strategies for improvement. Out of 752 cases, there was a substantial margin of error after the original codes were compared with audited codes. To identify specific underlying factors each case was analyzed. In reference to DRG changes, the study revealed an overpayment of at least 16%, with 56% of the error a result of poor documentation (Cheng, et al., 2009). Only 13% of the mistakes were related to actual coding errors with the remaining contributed to missing or inaccurate diagnoses in the medical chart.…

    The second section is steps 2 – 6, during the encounter. Step 2 is establishing financial responsibility. If the patient is insured, the health plan is examined for services covered, the billing situation, and any copay or coinsurance rules. If the patient has more than one health plan, it must be determined which plan is charged first.     According to the article three of the most common billing and coding errors are as follows double billing, typos, error in billing amounts. When a person is being double billed this means that they are getting charged for two of something ex: procedure or medication. As a solution I would suggest that one double checks their work with a fresh pair of eyes. Instead of billing out the file when finished set it down and start working on another file when one is done with that one pick the previous file back up to take a double look at it with a fresh pair of eyes. Second one is typos for example the dates of the hospital days could be incorrect or the number of hours one was in the operating room. The solution to this is similar to the double billing solution which is to double check your work as you are typing. If you think you saw a four double look to make sure that you did see a four and not a two. Over looking your work when finished comparing the original report to your bill. The third example of billing mistakes is amounts for example charging the patient of a six day stay but they only stayed four days or charging them for medication that they didn’t take.     In my opinion, HIPPA, ICD, CPT, and HCPCS influence each of the ten steps of the medical billing process in the way that when it comes to medical billing and the coding process, there is a special task that must be completed by the billing staff members of any medical facility, whether it is a small doctor’s office or a large hospital. All of these provide quality care while protecting the patients’ privacy and expediting the payment of services rendered.
